BID–Needham's Medical Records Department is dedicated to the proper maintenance of your medical records as well as to keeping your personal health information (PHI) private and secure in accordance with federal and state regulations.

Patients have a right to review or, for a fee, obtain copies of their medical records. To obtain a copy of a medical record, the patient is required to complete a medical authorization form. There is no charge for records delivered directly to another health care provider. The requester must be the patient or a patient's guardian/legal representative.

The average turnaround time to obtain copies of medical records is approximately 7 to 14 days depending on the number of records requested (one visit versus several visits) and the location of the records requested (some records are secured at an offsite location).
